Class Request Form

We're so excited to potentially host you at the Glass Patch to explore stained glass artistry. Please complete the information below. Once submitted, we will respond within 24 hours to confirm the time(s) and class. At that point, a link will be sent allowing you to pay for the class and finalize your reservation.
